- Sign in on the main sign-in sheet -- note the number.
- Take a clipboard with the same number.
- Keep the clipboard with you and take: (1) NYS risk release form, (2) MDSA application and (3) 2 membership cards.
- Read & sign the NYS risk release form AND fill out and sign the MDSA application (consider volunteering) and sign both cards.
- Pay the MDSA for: (1) $15.00 MDSA membership fee (refundable if you do not pass) and (2) $3.00 photos fee (if MDSA provides the photos).
- Sit with the Data Entry volunteer while your application is typed into the computer.
- Get your picture taken if you do not have 2 "driver's license size" photos with you (the charge for photos is not refundable if you fail the test).
- Cut photos, glue them onto 2 signed cards, and give them to the MDSA to laminate and hold onto. DON'T FORGET TO SIGN BOTH CARDS.
- Give clipboard with forms to MDSA Tester and take TEST: 20 lengths (10 laps) of the pool -- 1 crawl, 1 backstroke, 1 breaststroke, 1 side stroke, and 16 lengths any style -- plus 3 minutes treading water. THERE IS NO TIME LIMIT.
- Ask MDSA Test Team volunteer to sign off on your MDSA application and NYS risk release form.
- Return the clipboard to where you first picked it up, turn over your MDSA application and NYS release form to the "Banker" and get your membership card and the MDSA membership information.
- Please take the time to read the welcome information before you leave and ask an MDSA volunteer any questions while they are fresh in your mind.
